Affinity health Complaints - Outstanding invoice not paid

In October 2018 my daughter with the name of Nikki Rachael went to the dentist and the dentist submitted invoice to affinity health. The dentist was not paid and I was send invoice to pay. So I phoned affinity 3 to 4 times and ask what's happening and they said there was a problem but will resubmit. Till now nothing and I don't have cash to pay the dentist. I had to take my child of as she turned 18 and she was still covered till end of october 2018. I then got irritated and I canceled mine too. Please I need help. I can not pay this dentist while my daughter was covered.
